A: You will need to have your insurance card and photo ID with you. It is helpful for you to have your latest and most important medical records with you from your previous provider.
A: If you do not have copies of your previous medical records, you are more than welcomed to stop by our office and request a medical release form at the front desk. We will fax the release form to your previous medical provider and request your records. The Records Release form is also available on our website for you to print and fill out.
A: No appointment is necessary for routine injections. The injections are administered by the nurse. You can walk in during office hours and sign in at the front desk. Please inform the receptionist what injection you are in the office for. Cortisone injections MUST be scheduled, as they are administered by the doctor
A: Yes, you will need to come into the office for a urine test. You do not need to make an appointment. You can walk in during regular office hours and sign in at the front desk for the lab. Please let the receptionist know you are there for a urinalysis. We will test your urine in our lab and review results with the doctor before you leave
A: If you are scheduled for a fasting lab appointment, you will need to fast for 12 hours prior to your appointment time. We encourage you to drink plenty of water while you are fasting. You may also have black coffee and black tea ONLY. You cannot eat anything during the 12 hour window and please do not sweeten your coffee or tea with sugar or artificial sweeteners.
A: Yes, you may bring your lab order to our office and we can draw the blood in our lab. Please note there are some blood tests that we cannot perform in our office. In that case, we will send you to an outside laboratory closest to your hom
